Enrollment agreement and start date.
Emergency contacts and the authorized pick-up list, dated and current.
Immunization record or your state's exemption form.
Health history and physician / clinic information.
Allergy and medication plans, matched to what's posted in the room.
Signed consents: photos, transportation, sunscreen, screenings — whatever your program uses.
Incident and injury reports, signed by the family.
Developmental observations or screening results, kept confidential.
