Staff member · role · start date:

Application, résumé, and references checked.
Background-check clearance, with its renewal date on your calendar.
Qualifications: transcripts, credentials, CDA or degree as your state requires.
Orientation record, signed and dated.
Annual training log — hours to date against your state's requirement.
Current first aid / CPR cards, where required.
Health screening or TB test, where your state requires one.
Emergency contact for the staff member themselves.
